I use the information in my grade book to plan instruction and monitor student progress. I can easily identify problem areas and look for patterns of deficiency. With this information, I implement specific lessons that provide further instructional support during small-group rotations or individual conferences. After all, it hardly seems fair to assign a grade based on a limited number of test questions. (A student might receive a 50% score in the category of Main Idea, based on one incorrect answer out of a total of 2 questions.)
I use a combination of the following criteria for mid-quarter progress reports and quarterly report card grades:

Grading

Due to computer glitches and problems with our school server, I have, unfortunately in the past, lost student data. Therefore, each time a student is ready for the Success Zone, I print a hard copy of the Diagnostic Report, Individual Zone Report, and Progress Report. After recording the data, in my grade book, I place these reports in students personnel READ 180 file. Having a printed copy handy provides me the opportunity to refer to specific information as needed.

Teacher's Report Assistant is a free time saving software program for teachers who word-process student reports. It includes a comment library of ready-made statements available on the web (by subject and level) or you can write your own comments to suit individual students. It's quick and easy to use. It has significantly decreased my time spent writing anecdotal student reports.
